Hi, I’m Hannah Wagner, LCSW, and for the past 10 years, I’ve worked with older adults and their caregivers. In that time, I’ve seen firsthand the unique emotional and practical challenges that come with aging — challenges that deserve specialized care and attention. That’s why I created Roseville Elder Services: a private practice dedicated to meeting the distinct mental health needs of seniors and their families.
I support clients experiencing depression, anxiety, grief and loss, cognitive changes, chronic illness, and caregiver stress. My approach integrates Cognitive Behavioral Therapy (CBT), grief therapy, mindfulness, and a strengths-based perspective that honors each person’s life experience and resilience.
My work is informed by years of experience across the continuum of care — as a hospice social worker supporting individuals and families through end-of-life, as Director of Social Services in a skilled nursing facility overseeing psychosocial assessments, intake and discharge planning, and care coordination, and as the Older Adult Specialist Lead for the county helping older adults age safely at home or transition to higher levels of care. I’ve also worked closely with home health agencies, guided clients through advance care planning, collaborated with insurance companies to manage mental health benefits, and connected families with community-based resources.
My goal is to provide compassionate, specialized therapy that helps older adults and caregivers find clarity, connection, and renewed purpose through life’s later chapters. At Roseville Elder Services, I provide compassionate, specialized support for older adults, caregivers, and families navigating the many stages of aging.

I offer in-home and virtual sessions to make care more comfortable and accessible — meeting you where you are, both literally and emotionally.
When I am not working, I enjoy spending time with my family, exploring the outdoors, reading and cooking.
